Cancellations
If
you are unable to keep an appointment, we ask that you kindly provide us
with at least 48 hours notice. This courtesy, on your part, will make
it possible to give your appointment to another patient.

Billing/Payment
All billing inquiries are to be directed
to our main office at (631) 665-1330. Our staff is prepared to
answer any questions you may have regarding payments.
Most medical insurance plans generally
will not cover elective ophthalmic procedures such as LASIK refractive
surgery. We require payment in full before any LASIK
procedure. Forms of payment accepted by the office are cash,
personal checks, bank checks and money orders.
